    • Peabody have an exciting opportunity for a full time Health & Housing Coordinator role based at the Tunbridge Wells Hospital, Kent, also covering associated Cottage Hospitals and with an element of home visiting incorporated.  Ad hoc cover at Maidstone hospital may also be required.

      This role is seen as an integral part of the Integrated Discharge Team (IDT),  responsible for avoidance of hospital admissions wherever possible along with providing a timely proactive discharge planning service to ensure:

      • Support planning with best possible outcomes for the patient.
      • Timely access to range of community based health, social care services and voluntary sector services.
      • Optimum use of acute/community and social services resources.
